Complex Nursing Assessments for NDIS Participants
A complex nursing assessment is a detailed assessment completed by a Registered Nurse. It examines how a participant’s disability-related health needs affect their safety, independence, comfort and everyday life.
The word “complex” does not mean the participant is difficult. It means the participant may have several connected health and support needs that require careful nursing assessment and clinical judgement.
At Loica Care, we listen to the participant, consider the whole person and provide a clear report explaining what support may be required and why.
What does a complex nursing assessment examine?
Every participant is different. The Registered Nurse will only assess areas relevant to the participant’s needs, which may include:
- Personal care and daily living activities
- Mobility, transfers and positioning
- Continence and toileting
- Skin care, wounds and pressure injury risks
- Eating, drinking, nutrition and swallowing
- Diabetes management
- Seizure monitoring and management
- Respiratory support
- Medication-related assistance
- Communication and understanding
- Memory, cognition and decision-making
- Safety during the day and overnight
- Current support arrangements
- Support worker knowledge and training needs
- Equipment or assistive technology needs
- Family and informal support arrangements
- The participant’s personal goals
A complex nursing assessment is more than a list of medical conditions. It explains how the participant’s disability-related needs affect what they can do safely and independently.
What will the report explain?
Following the assessment, Loica Care prepares a detailed nursing report that may explain:
- What the participant can do independently.
- What they need help with.
- How their disability affects their daily life.
- What health and safety risks have been identified.
- What may happen if suitable support is not provided.
- What supports are clinically recommended.
- How much support may be required and how often.
- What skills or qualifications the person providing the support may need.
- Whether support workers require participant-specific training.
- How the recommendations connect with the participant’s goals.
- Whether referrals to other health professionals are recommended.
Recommendations are based on the participant’s individual assessment. They may include nursing care, support-worker training, care plans, continence products, pressure-care strategies, daily personal assistance, equipment trials or referrals to other professionals.
Helping prevent problems before they become emergencies.
Good NDIS nursing care is not only about responding when something goes wrong. It is also about identifying changes early and understanding how a participant’s disability-related needs may affect their health, safety, independence and everyday life.
A participant’s needs can change over time. A small change in mobility may increase their risk of falls or make transfers more difficult. Changes in continence may increase the risk of skin breakdown or infection. Difficulty eating or drinking may affect nutrition and hydration. Changes in a wound, seizure pattern, respiratory needs or medication management may also indicate that existing supports need to be reviewed.
A timely nursing assessment can help identify these changes, clarify the participant’s current support needs and determine whether additional nursing care, participant-specific training, equipment, clinical monitoring or referral to another health professional may be appropriate.

Can the assessment guarantee NDIS funding?
No. A complex nursing assessment provides clinical evidence and recommendations, but it cannot guarantee that the NDIA will approve a particular support or amount of funding.
The Registered Nurse makes the clinical recommendations. The NDIA makes the funding decision.
The NDIA considers whether a requested support:
· Relates to the participant’s disability
· Helps the participant work towards their goals
· Is likely to be effective and beneficial
· Represents value for money
· Works alongside informal, community and mainstream supports
· Is an NDIS support rather than the responsibility of the healthcare system
Read the NDIA’s reasonable and necessary support criteria.
The NDIS may fund certain disability-related health supports, including nursing, continence support, wound and pressure care, seizure management, diabetes support and participant-specific training, when the support meets the applicable NDIS requirements. Our 72-hour service target
We understand that participants and families may need timely clinical evidence.
Loica Care aims to:
· Arrange or commence the assessment within 72 hours of initial contact, subject to availability and receiving the required consent and information.
· Complete the written report within 72 hours after all required evidence has been received, including relevant charts, records and supporting documents.
We do not sacrifice clinical quality for speed. Additional time may be required when important information is missing, further assessment is needed or another health professional must be consulted.
