A Complex Nursing Assessment is a detailed clinical evaluation conducted by a Registered Nurse to identify your health needs, functional capacity, and NDIS support requirements. Our Registered Nurses conduct complex nursing assessments for NDIS participants across Melbourne who require a detailed clinical picture to support plan reviews, specialist referrals, or new support requests. We assess current health status, medication needs, risk factors, and daily living requirements in a single comprehensive visit.
The assessment findings are documented in a clear, evidence-based report that can be used to support NDIS plan reviews, funding requests, and coordination with your treating team. We write in plain language so participants, families, and support coordinators can all use the report effectively.
