Wound and Pressure Care Assessments for NDIS Participants
A wound and pressure care assessment is a detailed clinical assessment completed by a Registered Nurse.
It examines the wound, the surrounding skin and the factors that may be causing the wound or delaying healing. It also considers how the participant’s disability affects their ability to prevent, monitor or manage wounds and pressure injuries.
A proper wound assessment involves much more than taking a photograph, measuring the wound or selecting a dressing.
At Loica Care, we consider the whole person, including their health, disability, mobility, environment, daily support, risks and personal goals.
Who may benefit from an assessment?
A wound and pressure care assessment may be suitable for a participant who:
- Has a slow-healing, chronic or recurring wound
- Has a pressure injury
- Is at increased risk of developing a pressure injury
- Has repeated skin breakdown
- Has reduced movement or reduced sensation
- Spends extended periods in bed, a chair or a wheelchair
- Cannot reposition independently
- Cannot inspect, dress or monitor their wound safely
- Requires help with pressure care or dressing changes
- Has different people providing wound care without a consistent plan
- Requires participant-specific instructions for nurses or support workers
- Needs clinical evidence about ongoing disability-related wound or pressure care
New, infected, unexplained or rapidly worsening wounds may require prompt assessment by a GP, hospital or other medical service.
What does the Registered Nurse assess?
The assessment is individualised according to the participant’s wound, disability and clinical needs.
The history of the wound
The nurse may examine:
- When and how the wound started
- How long the wound has been present
- Whether it is improving, deteriorating or remaining unchanged
- Whether the wound has occurred previously
- Previous dressings and treatments
- Hospital admissions or procedures
- Relevant medical investigations
- Previous wound assessments
- Involvement of the participant’s GP, podiatrist, vascular specialist or other clinicians
The wound and surrounding skinWhere clinically appropriate, the nurse may document:
- Wound location
- Length, width and depth
- Wound-bed tissue
- Wound edges
- Surrounding skin condition
- Exudate type and amount
- Odour
- Pain or discomfort
- Bleeding
- Swelling
- Undermining, tracking or cavities
- Possible infection or deterioration
- Pressure injury stage or other appropriate clinical classification
- Changes since previous assessments
Clinical photographs may be taken only after informed consent and in accordance with privacy requirements.
Does the assessment guarantee NDIS funding?
No.
A Registered Nurse provides clinical findings and recommendations. The NDIA makes the funding decision.The assessment must show how the requested support:
- Relates to the participant’s disability
- Addresses an identified functional or safety need
- Supports the participant’s goals
- Is expected to be effective and beneficial
- Represents value for money
- Is appropriately funded by the NDIS rather than the health system
When should urgent medical help be obtained?
Loica Care is not an emergency service.Prompt medical assessment is required if there is:
- Uncontrolled or severe bleeding
- Rapidly increasing redness, heat or swelling
- Pus or a significant change in wound drainage
- Fever, confusion or the person appears seriously unwell
- Sudden or severe pain
- Rapid wound deterioration
- New black or significantly discoloured tissue
- A cold, pale or blue limb
- A deep wound or major injury
- Any other serious or sudden concern

Our 72-hour service target
For non-emergency referrals, Loica Care aims to:
- Complete the assessment within 72 hours of initial contact, subject to participant availability, location, clinical urgency, consent and receipt of essential information.
- Complete the written report within 72 hours after the assessment and receipt of all required clinical evidence.
More time may be required if serial wound reviews, product trials, medical investigations, specialist advice or additional information are needed.
