At Loica Care, our Registered Nurses provide professional, person-centred nursing assessments and clinical care in the community.


We work with Support at Home providers, families, carers and self-managed clients to provide clear clinical information and practical recommendations that help ensure care is appropriate, coordinated and responsive to the individual.

Whether you are a Support at Home provider seeking additional clinical support or a client managing your own care and wanting to understand how your budget can best support your needs, we can help.

What can an Aged Care Nursing Assessment look at?

Every person is different. Our Registered Nurse will tailor the assessment to the individual's health, circumstances and goals.


Depending on the person's needs, an assessment may consider:

  • Overall health and wellbeing
  • Mobility, transfers and falls risk
  • Personal care and activities of daily living
  • Continence and toileting
  • Bowel and bladder management
  • Skin integrity and pressure injury risk

Our assessment looks beyond a diagnosis. We consider how a person's health and changing abilities affect their everyday life and the care they require to remain safe, comfortable and as independent as possible.

Why is a nursing assessment important?

It can be difficult for families and care providers to know whether a person's current supports are still appropriate.

At Loica Care, we help clients understand their clinical and nursing needs so they can make informed decisions about their care.

Our Registered Nurse can identify:

  • What nursing care may be required
  • What assistance may be needed with daily activities
  • How frequently care may be required
  • What risks should be monitored
  • Whether existing supports remain appropriate
  • Whether additional services or referrals may be beneficial
  • Whether further assessment by another health professional may be appropriate

Loica Care can partner with Support at Home providers to provide specialist nursing services for clients in the community.


Our Registered Nurses can assist with clinical assessments, ongoing nursing care, care planning and specialised areas of need.

We can work alongside your existing care team to provide additional clinical oversight while keeping the client at the centre of care.

Helping prevent problems before they become emergencies.

Good aged care nursing is not only about responding when something goes wrong.

Early identification of changes can help support better care planning and may reduce the risk of complications.

For example, a small change in mobility may increase falls risk. Changes in continence may increase the risk of skin breakdown. Reduced appetite may indicate nutritional concerns. A wound that is not healing as expected may require a review of the current treatment approach.

A timely nursing assessment can help identify these changes and determine whether further action is required.

Our 72-hour service target

We understand that participants and families may need timely clinical evidence.

Loica Care aims to:


  • Arrange or commence the assessment within 72 hours of initial contact, subject to availability and receiving the required consent and information.
  • Complete the written report within 72 hours after all required evidence has been received, including relevant charts, records and supporting documents.


We do not sacrifice clinical quality for speed. Additional time may be required when important information is missing, further assessment is needed or another health professional must be consulted.
